ACC/AHA/AATS Class IIa prophylactic lumbar CSF drain for high-risk endovascular aortic repair; target MAP 85-100, ISP <15, SCPP >70-75 mmHg.

Prophylactic lumbar CSF drain (14G Tuohy at L2-L3/L3-L4/L4-L5) maintains SCPP > 70-75 mmHg during and after EVAR by draining CSF in 10 mL/h aliquots to keep ISP < 15 mmHg while MAP is driven to 85-100 mmHg with vasopressors.
